XXXX XXXX XXXX Policy # XXXX Retired Employees of XXXX XXXX CountyXX/XX/XXXX Teeth cleaning with XXXX XXXX XXXX XXXX.
XX/XX/XXXX XXXX issued check to XXXX XXXX {$90.00} for service. XXXX XXXX does not receive check.
XX/XX/XXXX I obtained copy of cashed check from XXXX and forwarded it to XXXX XXXX. The check had been cashed by California Bank and Trust, XXXX XXXX. XXXX XXXX XXXX XXXX, XXXX XXXX, CA XXXX. XXXX XXXX XXXX filed an affidavit with XXXX for non receipt of check.
XX/XX/XXXX After checking with their security department, the California Bank and Trust manager instructed me to have XXXX XXXX file another affidavit with their bank. He filed the XXXX affidavit.
XX/XX/XXXX I met with XXXX XXXX XXXX, Manager XXXX ( XXXX ) to follow up on the status of the affidavit. She determined that XXXX had failed to notify us that no action was being taken on the affidavit because XXXX XXXX was not a customer of XXXX. She found the cashed check electronically and their customer who had cashed it. She would not release the name of the customer to me. Since XXXX had accepted the check without verification, she agreed that the transaction was either an error or fraudulent. I requested that XXXX write a check for {$90.00} to XXXX XXXX and debit their customer 's account. She refused.
XX/XX/XXXX I have had many conversations with XXXX XXXX, XXXX XXXX XXXX, XXXX ext XXXX. As of this date, action on XXXX XXXX 's affidavit is apparently still pending. She was unable to provide a case number from XXXX XXXX bank who issued the check.
