Fortiva issued a credit card in my name to someone else and the account is delinquent. I contacted Furtive and to date they have taken no action, indicating over a week ago that they would send me an affidavit by mail to complete. My reports to XXXX show no activity to resolve false addresses and phone numbers used in addition to the false credit.
