The Original creditor failed to bill the insurance company for XXXX visit. I have provided proof that I had valid coverage to both the Original Creditor and the Collection Agency. I was told by XXXX that it is too late to bill the Insurance therefore I am responsible for the debt. This debt is being negatively reported on my Credit File with all XXXX Credit Bureaus. I am requesting a new investigation and have this item removed permanently.
