XXXX XXXX is my Health Insurance and Health Services company. I had a procedure that came with {$100.00} copay. I paid by charging my credit card in advance of the procedure at their prompting. The procedure and payment were XX/XX/2015.I received a collection letter for the item on XXXX XXXX, 2015 from their collection agent AMO Recoveries . I was never contacted by XXXX about the lack of payment prior to this letter from the collection Agent.
Not only was the payment in advance of service, XXXX never notified me they had no record of payment.
