My name is XXXX XXXX, and I am experiencing a horrible billing problem for services that were provided to me on XXXX XXXX of this year in XXXX XXXX XXXX facility. First, let me assure that I understand that XXXX XXXX 's outsource the XXXX services and that the billing issue is not directly with the Hospital.
I walked to the XXXX that day, with pain on my neck after being in a car accident the day before. I fill out my intake sheet completely, providing my address and SSN. A nurse took my vitals, I was checked by a doctor ( I assume ) for less than 5 minutes, and I was given a shot for the pain, being release about 30-40 minutes after my arrival. ( No XXXX, XXXX or any kind of test was performed to me ) On Friday, XXXX XXXX, a little over a month after my XXXX visit, I received a letter from a COLLECTION COMPANY. I called this company and they advised me that if I did not pay immediately {$810.00} dollars they were going to ruin my credit. I was and I am very upset. I did not have the chance to see what I am being charged for. What if there is a mistake? What if they charging me more or for services that were not provided to me? I understand that XXXX services are expensive, but {$810.00} just for the 5 minutes the Dr. saw me seems a lot. I was denied my right to see my bill and my good name is being besmirch unnecessarily.
I try to contact XXXX XXXX XXXX billing department in XXXX. I have an unpleasant experience talking to a lady that threaten me to make my life a living XXXX If I continued to talk about my situation. The Fair Debt Collection Practices Act, has been violated, and I am really concern how this is going to affect my credit. The person I spoke to from XXXX billing in XXXX told me that all this happen because XXXX XXXX 's did not provide my information.
Why XXXX XXXX 's did not supply my information? What happen with that paperwork that I filled out? Is there a chance that a person have access to my name, address, SSN, and other personal information? I am so scared, Identity theft is a really looming problem to me right now.
