The account listed as an open collections account listed under XXXX AD in the amount of {$860.00} is incorrect. This account should have never been placed in collections to start with. Also, it was disputed in XX/XX/XXXX 2014 and to be removed at that time. As you can clearly see from the attached documentation, I do not, nor have I ever owed this money. My health insurance plan at the time offered a {$1000.00} PPO discount with XXXX XXXX XXXX XXXX original creditor ). This left me a balance of {$320.00}. I was told XXXX that I only needed to pay {$300.00}. My cancelled check is attached as well. Please remove this account from my credit report.
