I went to XXXX to received the medical emergency help in XXXX 2015 due to the work injured and I had the claim number which I provide to hospital, but the bill was sent to me.

I tried calling ( XXXX ) XXXX number in hoping to talk with XXXX ( the actual bill : proof file attached ) trying to explain to them to what was going on, but this number would never be working. There were no chance to talk and correct this matter.

So, I had to forward the bill to my attorney in XXXX 2015 ( please see proof file attached ) No further bills have sent to both my attorney and I after XXXX 2015.


Bill was not paid until I acknowledged the alert from my credit report ( on the night of Friday, XXXX XXXX, 2015 ) where my point dropped down XXXX points. After I know the problem, I took care of the bill myself ( payment posted on my account on Tuesday, XXXX XXXX, 2015 ).

My attorney made the informal conference to XXXX agent ( please see file attached ) XXXX did not take care this bill. That was the reason why my credit score has dropped XXXX points because the collection action was put under my name.

I need XXXX insurance explain in writing where I report to credit bureau that it was not my false.
