Narrative (may be redacted).

XXXX XXXX failed to correctly file medical bill with XXXX. Filed with XXXX in wrong name. Account was sent to a collection agency Receivables Management Systems. I made calls to XXXX XXXX and learned they had not put my name as it appears on my XXXX card and XXXX would not pay the bill. Consequently, due to XXXX not paying our other XXXX insurances rejected the claim as well.

We have XXXX and XXXX. However, if XXXX denies a claim for any reason it is rejected by the secondary and third insurance.

I have also learned even though I spoke to XXXX XXXX in XXXX, XXXX, the file was not resubmitted until XXXX XXXX. This caused the Receivables Management Systems to continue to carry the bill as a collections.

I spoke to XXXX at Receivables Management Systems before the account was added to my credit file and let her know that the bill had not been properly submitted for payment. She said she would wait to hear back from XXXX XXXX. But the account was still placed in collections and sent to the credit bureau.
